Rhode Island: Submit the Independent Contractor Form on the RI DLT website, then check the box below once complete.

Connecticut: Please fill out and submit the CT Form 75 to us directly on this platform.

These forms confirm your independent contractor status and that you’re not eligible for workers’ compensation through our company. We know this is a hassle but thank you for your quick attention to this — it helps us stay fully compliant!
